HMB after 40 for building and keeping muscle
If you already lift weights and eat enough, HMB is optional. Research has not shown a clear, reliable boost in muscle or strength for people in that situation. You can skip it without feeling you've missed a basic part of getting stronger. That is the practical message from research in younger exercisers and older adults who exercise.
The decision may be different if you are losing muscle during a long period of rest or illness. In that case, ask your care team about food, activity and recovery. HMB may be worth discussing as part of that plan.
This is general education. Talk with your medical clinician about new weakness, muscle loss you can't explain or hormone supplements.

What HMB can and cannot do
Your body makes HMB when it breaks down leucine. Leucine is an amino acid, one of the building blocks in protein. A supplement gives you more HMB than your body normally makes from food.
Muscles build new proteins and break down old ones all the time. HMB can affect that process. Small lab studies in healthy young men found more protein building and less breakdown over a few hours. How HMB affects muscle protein
This helps explain the word anabolic on a supplement label. It means building tissue. But a change that lasts a few hours does not prove you will have bigger muscles or lift more weight months later.
HMB also lacks the full set of amino acids your muscles need. It cannot make up for too little food or protein.
If you already lift regularly
Before you buy HMB, check your training plan. Are you doing your workouts often enough to follow it? Are your weights or repetitions going up over time? Can you stick with the plan when progress feels slow?
Research has not shown a reliable extra benefit from adding HMB to exercise. Some people might benefit, but there is little reason to expect a big change. Review of HMB evidence
If money or time is tight, focus on a training routine you can manage and enough food. You don't need every supplement that has a sensible theory behind it.
Be careful with striking before and after claims. Some HMB studies reported large gains that other researchers have questioned. Some studies also have links to supplement companies through funding, jobs or patents. Those ties do not prove the findings are wrong. They do make it important for other research teams to check the results. The ISSN review supports HMB overall but also describes these disputes.
When HMB is worth discussing with your care team
Losing muscle while unable to train is different from wanting faster gym results. If illness, long periods of rest or poor appetite are holding you back, ask your clinician or dietitian for help.
HMB might help limit lean mass loss during bed rest, but the evidence is not firm. A small study found less loss of lean mass, which means body tissue other than fat. The result depended in part on leaving out one unusual result. It did not prove better strength or daily function. Bed rest research
The results are mixed for sarcopenia too. This is a condition that involves loss of muscle and how well it works. Some research found that HMB with exercise helped walking speed, but did not reliably improve muscle mass or strength. HMB and sarcopenia
If your care team suggests HMB, ask what it should help you do. Getting out of a chair more easily is a useful goal. Ask how you will track progress and when to review the plan.

Rebuilding after illness without chasing your old workouts
After a flare or a long recovery, a basic workout may feel hard. Start with a plan that fits what you can do now.
Crohn's disease can make it harder to eat enough or absorb nutrients. Tell your care team about new weakness or weight loss you did not plan. An IBD focused registered dietitian can help assess your food needs. IBD means inflammatory bowel disease. HMB does not replace that care. Crohn's and nutrition concerns
Fatigue, pain and urgent bathroom trips can get in the way of exercise. Tell your care team what is hard instead of just pushing through. Much of the exercise research in Crohn's and colitis involves people with mild disease or symptoms that are under control. It does not give you the all clear to train through a severe flare. Being active with Crohn's or colitis
Once you have advice on safe activity, a coach can help you plan sessions around it. If you have another autoimmune condition, follow advice for that condition. This article is not a treatment plan for autoimmune disease.
What to check on the label
If you and your clinician decide to consider HMB, read the full label.
The two main forms are calcium HMB and HMB free acid. One form may enter the blood faster, but that does not prove it builds more muscle or strength. There is not enough direct research to call one form clearly better. HMB forms and absorption
Many studies use about 3 grams a day. That is a study amount, not a dose recommendation for you. There is no clear evidence that taking more will help you more. Check how much HMB is in a full serving. One serving may mean several capsules. NIH HMB overview
Calcium HMB adds calcium to your intake. Some products also add vitamin D. Check other supplements you take so you don't overlook those amounts. Studies of HMB with vitamin D cannot show how much benefit came from HMB alone. They also do not prove that the mix adds to a good lifting plan. HMB with vitamin D research
If a label says “HMB + D,” check what D means. Vitamin D and DHEA are very different ingredients.
My experience with HMB and DHEA
While taking HMB and DHEA together, I noticed more muscle growth, increased strength and better recovery. That's my personal experience, not a controlled comparison. It can't establish that the combination caused those changes, works better than either supplement alone, or will produce the same results for someone else.
Your body can turn DHEA into hormones such as testosterone and estrogen. This gives it different effects and risks from HMB.
DHEA is not a reliable shortcut to better gym results. Studies have found some added benefits and no added benefit. Higher hormone levels do not always mean more strength. A small gain in lean mass is not the same as proof of more muscle or better performance. DHEA body composition review
The research reviewed here does not establish a benefit from taking HMB and DHEA together. Our search found no human trial that tested muscle or strength gains from the pair against each supplement alone and a placebo. A placebo has no active ingredient. That does not prove no such study exists. It does mean we cannot call the pair a proven way to get better results.
Talk with your clinician before trying DHEA, alone or with HMB. The DHEA after 40 guide explains more about the possible benefits and risks.

Safety before you buy
HMB has generally caused few problems at the amounts used in studies. Short studies and some research lasting a year are reassuring. They do not prove it is safe for every person, medicine or length of use. NIH safety discussion and longer term HMB research
Bring the product label to your clinician or pharmacist if you take medicine or have a health condition. Get advice during pregnancy or breastfeeding too. Results from other adults may not apply to you.
DHEA needs extra care. It can cause acne, oily skin, unwanted hair growth and mood changes. Those changes can include mania, a serious state of unusually high or irritable mood, in people at risk. DHEA may also lower HDL, often called good cholesterol. Mayo Clinic advises against DHEA during pregnancy or breastfeeding, or if you have cancer that is sensitive to hormones. DHEA safety guidance
Check for drug interactions too. Memorial Sloan Kettering warns against taking DHEA with tamoxifen.
If your sport follows World Anti Doping Agency (WADA) rules, DHEA is banned both in and out of competition. A prescription alone does not make it allowed. You must meet the rules for a medical exemption. Being banned is not proof that it gives useful gains. USADA athlete guidance
The FDA regulates supplements, but it does not approve them for safety and effectiveness before sale as it does new drugs. A product on a store shelf is not proof that its claims are true. FDA supplement guidance
Deciding whether HMB belongs in your plan
For healthy adults who train often and eat enough, HMB is optional. The evidence does not make it a priority after 40.
If you are losing muscle or struggling to rebuild after illness, get help finding out why. Your clinician or dietitian can decide if HMB fits your care plan. Don't put off that visit while trying another supplement.
Track your exercises, weights and repetitions. Judge progress over several weeks, not one workout. If you change your training, food and supplements at once, you won't know which change helped.
The Steadfast guide to strength training supplements after 40 puts HMB in context with other options. You can make a sound choice without treating supplements as a must.
If you want help building a training plan you can follow and adjust, talk with Steadfast about your strength and mobility goals. Questions about unexplained muscle loss, hormone testing, or DHEA treatment belong with your medical clinician.
Common questions
Is HMB worth considering if I already lift regularly?
HMB is optional if you train regularly and eat enough. Research hasn't shown a reliable extra benefit from adding it to exercise, although some people might benefit. Put your training routine and food needs first, especially if money or time is tight.
Does taking HMB with DHEA produce better results?
The research reviewed here doesn't establish that. My experience can't show that the combination caused my gains, works better than either supplement alone, or will work the same way for you. Read the experience and its limits, and talk with your clinician before trying DHEA alone or with HMB.
What should I do if I'm losing muscle after illness?
Ask your clinician or dietitian to help find out why, especially if you have new weakness or weight loss you didn't plan. Don't delay that care while trying another supplement. Once you have advice on safe activity, a coach can help you build training around your current starting point.
